Application for US  Passport or Registration

APPLICANTS WHO HAVE HAD A PREVIOUS U.S. PASSPORT:
f your most recent passport was issued less than 15 years ago and you were over 16 years old at the time of issuance, you may be eligible to use Form DS-82 (mail-in application).  Please inquire about eligibility when you apply or visit the website as listed above.  Address any requests for a passport amendment, extension of validity, or the addition of visa pages to a Passport Agency or a US Consulate or Embassy abroad.

FIRST TIME APPLICANTS:
Please complete and submit this application in person.  Each application must be accompanied by:

1-     PROOF OF US CITIZENSHIP
State issued birth certificate (PLEASE NOTE:  Hospital issued birth certificates are not permitted.  It must have the state seal on the certificate.)

2-     PROOF OF IDENTITY
You may use a previous US passport, Certificate of Naturalization, Certificate of Citizenship, driver’s license, or government employee identification card or pass.

3-     TWO PHOTOGRAPHS  (Passport photos)

4-     FEES
      The passport processing fee is $55.  In addition, a fee of $30 is charged for the execution of the application.  Your passport will be valid for 10 years from the date
      of issue  except where limited by the Secretary of State to a shorter period.

For faster processing, you may request Expedited Service.  Expedited requests will be processed in three workdays from receipt at a Passport Agency. 
The additional fee for expedited service in $60.  Expedited Service is available only in the United States.

 WHERE TO APPLY
You may go to the Rocky Mount Public Library to apply for the passport.  Please remember we need your passport ASAP.  Certain trips require us to apply for a visa.  We need every passport on the trip to apply for the visa.  It is vital you apply this week to receive your passport in sufficient time to apply for the visa.  We will take care of applying for the visa if one is needed for the trip.
